The Diabetes Prevention Program: A Collaborative Approach to Community Health & Well-Being Ashley Brage & Andrea M. Landon DO The prevalence of Diabetes and Prediabetes continues to rise across Michigan. Providing patients with the education and ongoing support they need to successfully manage their health can be challenging due to limited time and competing priorities in clinical practice. The YMCA’s Diabetes Prevention Program, a CDC Fully Recognized lifestyle intervention program, offers an effective solution for individuals at risk of developing Type 2 Diabetes. Through evidence-based behavior change strategies, the program helps participants lose weight, lower their A1C, and increase physical activity. This presentation explores the program’s structure, eligibility criteria, outcomes, and participant success data, while highlighting the physician perspective and offering actionable strategies for integrating the program into patient care.
